About the company

Our client is a Series B healthcare AI company in San Francisco. Their platform lets health systems build and run AI agents for clinical and operational work, and it is already deployed inside large US academic health systems. The company grew quickly over the last year and the engineering team is small relative to the number of deployments in flight, so a forward deployed engineer here is working on live clinical workflows from the first week rather than on internal tooling.

The role

This is a founding-level, high-ownership forward deployed seat for someone with roughly one to six years of experience. You are the link between clinical stakeholders and a production AI platform: you sit with physicians, hospital IT and clinical champions, work out what is actually broken, and then build and ship the agent that fixes it. The loop is deliberately short — a physician flags something and the platform is different within days. It is a hybrid role, four days a week in the office, because the work depends on being in the room with both the clinical customers and the core engineering team.

What you'll do

  • Meet directly with physicians, hospital IT and clinical champions to understand their workflows and pain points
  • Translate loosely defined clinical ideas into concrete, production-ready AI agents on the client's agent platform
  • Build and deploy agents end to end, including the evals that prove they work as intended
  • Keep deployed use cases delivering value, iterating on agents against clinical guidelines and operational feedback
  • Own the full loop — from a clinician flagging something broken to the platform being fixed, in days

Nice to have

  • Has scoped, built and deployed AI agents or LLM-powered workflows for enterprise customers
  • Early employee or founding engineer at a venture-backed startup
  • Healthcare or health-tech background — FHIR, EHR integrations, clinical workflows
  • Forward deployed experience at a company built around that model
  • Familiarity with Rust and infrastructure tooling such as Kubernetes, Helm and AWS

Tech stack

Python on the backend with React and TypeScript on the front end; LLMs and AI agents; FHIR and EHR integrations; Rust, Kubernetes, Helm and AWS around the platform. The client cares much more about production judgement and customer instinct than about matching this list exactly.

Compensation & logistics

  • $180k–$285k base + competitive equity
  • Hybrid in San Francisco — four days a week in the North Beach office, with relocation support offered
  • Open to candidates who need no sponsorship, to visa transfers such as OPT and H-1B, and to new visa sponsorships including new H-1B and TN
  • Customer-facing work with physicians, hospital IT and clinical champions is the core of the job, not an occasional extra
  • The client is hiring several engineers into this role
